The cover artist for the first issue of CASUAL HEROES wasn't Allred, but the great Steve Rude. I will go look in a few quarter bins for a copy and post a JPEG of the cover and maybe some of the ugly interiors.
But in the meantime . . .

Okay. SOURCE Comics had a copy for right around cover price. And I may be wrong, but it looks to be in either very fine or near mint condition.

Why does Dave Johnson draw the covers for 100 BULLETS?
I had artists whose work I admire draw the covers for the series.
The second issue had a cover by Paul Pope.
The third issue had a cover by Kyle Baker.
The fourth issue had a cover by Jay Stephens.
Howard Chaykin was slated to be the cover artist for #5, but the series was discontinued due to a contract dispute before things got that far. Only one issue saw print, but four issues saw various degrees of completion.
Image agreed to publish CASUAL HEROES as part of an imprint packaged by Motown Records. I guess they liked the material. Or they relished the opportunity to partner with the record company.

Oh, I think I must not have understood what you meant in your last post.
The cover has the credit McCARTHY/STANGELAND, which was, and I guess still is, confusing becuase it looks like I am the writer and John Stangeland is the artist, when actually Stangeland only co-inked the book with me. The page above and the page below this post are written and penciled by me, inked by Stangeland.
Sorry for the confusion. This art is from when I was just breaking into comics and as much as I love it, looks awful to me now.
